1. Meet with your academic advisor about your plans
2. Obtain the required multi-part form from your advisor
3. Complete and sign the form, make sure to provide a course description from the institution you are requesting transfer credit.
4. Bring the form and the course description to the Mathematics Department Office, Gibson 424

Your request will be considered by a member of the department’s faculty. Please allow 3 to 4 days for this review. Once a decision has been made regarding your request, you will be contacted. Once contacted, you can then:

5. Pick-up the form from the Math Office, Gibson 424
6. Return the form to your academic advisor, if you do not bring the form to your academic advisor, no credit can be given.
